
Look at the current situation.
What is already happening?
Is anything urgent?
If there isn’t a crisis, what parts of everyday life are the most difficult?
Plan for one year.
What can realistically and practically change in the next year?
Everything beyond that is a vision, not a plan.
Consider the needs of everyone involved.
What does this mean for the person being supported?
What does this require from the caregiver?
Then ask:
- Why are we doing this?
- Are my expectations appropriate?
- Could it look differently?
- Do they understand the why’s and how’s?
- Do we have cooperation and participation?
- What one part can improve right now?
